Just as your caregivers have responsibilities, you also have responsibilities as a partner in the process of improving your health. You and your family have a responsibility to:

• Provide accurate and complete information regarding present and past illnesses, hospitalizations, medications and all other health matters
• Follow the treatment plan recommended by your healthcare provider
• Accept accountability for refusing treatment or not complying with the instructions of your healthcare provider
• Assure that financial obligations for your healthcare are fulfilled as agreed
• Follow PMCA’s rules and regulations affecting patient care and conduct
• Be considerate and respectful of the rights of other patients and clinic staff and property
• Be conscientious and considerate by calling into the office on an urgent basis only
• Request refills for prescription medication well in advance of running out – please do not wait until the last minute.
